According to Article 15 of the Data Protection Regulation (GDPR), you have the right to receive a register extract from Humana that shows which data we process about you. To request a register extract, you need to fill in this form.
In order for us to be able to ensure that the register extract is received by the right person, the register extract will be sent to your civil registration address. If you wish the information to be sent by email, you must visit one of our offices to identify yourself. This is to ensure that information about you is sent to you and not to anyone else.
The processing time may vary, but you should have access to your data no later than 1 month from the receipt of the request. If it is a request of a complicated nature, the deadline can be extended by two months. We will inform you if the deadline needs to be extended and the reason for this.
Please note that if you wish to obtain copies of your medical records, this form should not be used. Contact directly the business (Customer manager, HVB, nursing home) that applies.
The form is sent to data dataskyddsombud@humana.se.
If you want to mail the request to us, sign the document and write today's date after you print it.
